The password ensures that only students in the listed class will provide feedback, and that only one evaluation form per student will be accepted. Entries can only be accepted until the expiration date above.

HOW WE MAINTAIN YOUR ANONYMITY:

Our program compares your password to a list of valid passwords generated for the class. There are no names on this list. Once you enter the system, you must fully complete your entry, as the password cannot be used again for this instructor's evaluation. All passwords are automatically erased from the list. Your password is not recorded or linked in any way with the electronic form you submit. We have no way of determining which students submit evaluations, and we of course will make no efforts to determine the identity of any respondent. The instructor you are evaluating will not see your comments until after final grades for the course have been submitted.
