About Bubble Sheet Forms

This part of the website helps faculty to check and score blue bubble forms created by a printing house, either the narrow (single/double-sided with 50 rows per side) form or the wide one (100 rows on a single side). Only single-choice responses are recognized as well as the marks corresponding to the ID digits. Any other option on the form is ignored.
Either a soft pencil or a pen may be used; darker bubbles are better recognized. Errors can be corrected with correction fluid or an eraser.
To score the test responses, perform the following steps:
  1. Scan stacked responses into the PDF file using the departmental copier feature "Scan and Send" and "Send to Myself" after that. Keep the answer keys on the top of the stack. 90 or below responses per batch/file are recommended.
  2. Save PDF file received by email (You may use any method suitable for you to create the file with scanned responses). Go to the Upload Form, select your type of form, PDF file, and click Upload
  3. Wait for the scoring to finish. In case there are problems with the uploaded file, error messages will appear on the page.
  4. Upon completion, download the recreated PDF file with the scores and flagged responses. Download the Excel file with the scores and some test statistics and correlations.
The PDF file contains the list of the names captured from the namebox of each test, test number, and score. The name is the link to the corresponding test page within the file. Special attention is required to the red scores, referencing the tests where some of the responses are missing or were not recognized.
In case of an incorrect response, the correct letter is printed on the same row. A prepended question mark indicates an unrecognized or blank response.
The Excel file contains 5 worksheets with the scores of each student, total test statistics, statistics of responses on each question, and a matrix of the students' responses.
The ID and Score columns can be exported/copied as a separate CSV/TXT file and imported into many Learning Management Systems (e.g., Brightspace) without manual input of every result.
This part of the website helps faculty to create and score custom single-sided bubble forms with a preferred number of responses per row. Response blocks are literal (A, B, C, ...), numerical (1, 2, 3, ...), and boolean (T, F), with the possibility of multiple-choice responses and different number of points per correct answer.

Go to the Custom Forms page and either start the new form or enter the existing form number to load its configuration and to create an updated version of the form. Form Designer has the following blocks that can be added to the form layout:
  • Literal bubbles (A, B, C, ...), resize to change the number of rows and columns
  • Numerical bubbles (1, 2, 3, ...), resize to change the number of rows and columns
  • Boolean bubbles (T, F), resize to change the number of rows only
  • The Information block inserts spaces for name, class, and date
  • The Score block is the placeholder for the final score
  • The student ID block bubbles can be recognized and assigned to each test result
  • The Text block is for any text to be printed on the form
Access to the form can be protected with the password (Pwd). Any form has to be saved before downloading or printing the generated PDF file.
Scale the page in the Print Dialog to fit the printable area. Corner squares are important for the correct orientation of the scanned responses and should be present on each test page.
